Pay Period Survey

Directions:

1. Please complete the tabs for Parish/School/Agency Number, Name, etc.
2. Select the “Pay Frequency” from the drop-down menu in each applicable section (salaried, hourly, contracted principals and contracted teachers).
3. Enter the pay date information for the last two payrolls:
        a.   The “pay date” is the date of the payroll check.
        b.   The “pay period” is the actual dates that the paycheck covers.
4. Please note that the pay periods may differ between the salaried employees and the hourly employees because the salaried employees may be paid through the date on the paycheck while hourly employees may have a holdback period.
5. You do not need to include the Priest Payroll on this survey.
6. Please reply by Tuesday, March 4th.
