Paylocity Contact Survey

We are working with Paylocity to prepare for our initial training sessions. They are requiring that we provide some additional information on individuals who will be approving timesheets and/or processing payroll on the new Paylocity system. Please note due to security requirements, personal cell phone numbers are needed. Do not list your parish/school/agency phone numbers.

This survey was not sent to priests or principals.  Please consult with them, if it pertains to your location, and submit only 1 survey per location.
